Clear Sign WWE Star Roman Reigns Is Injured

Roman Reigns is set for a massive bout against Jacob Fatu at WWE Clash in Italy. However, he may not be a 100% for the upcoming premium live event.

The OTC challenged CM Punk for the World Heavyweight Championship at WrestleMania 42, earning the opportunity by winning the 2026 Men’s Royal Rumble.

After an intense back-and-forth encounter, Reigns managed to outlast The Second City Saint and walked away with the gold.

Injury concern for WWE star Roman Reigns

Post-The Show of Shows, Jacob Fatu has proven to be Roman Reigns’ biggest opponent for the World Heavyweight Championship. The two clashed at Backlash 2026, and Reigns successfully retained his title.

The Samoan Werewolf, however, had the last laugh as he launched a brutal assault on his real-life cousin. On the May 11 edition of Monday Night RAW, Fatu continued his destructive ways. He put Roman Reigns and The Usos through the barricade to make a massive statement.

In footage that emerged after Fatu’s actions, it became clear that The OTC injured his elbow. The volatility between Reigns & The Usos and Jacob Fatu stayed there for the following week as well, leading to Jacob Fatu challenging Roman Reigns for the World Heavyweight Championship under Tribal Combat rules. Reigns accepted without much hesitation.

On the go-home episode of RAW before Clash in Italy, The OTC and The Samoan Werewolf confirmed their encounter by signing the official contract.

Roman Reigns pulled off a last-minute twist on Jacob Fatu, saying that if the latter lost, he would have to serve under Reigns. If Jacob Fatu managed to pick up the victory, then he would become the next Head of the Table.

Another thing that became apparent during the show was that The OTC sported a black elbow brace. It looks like Reigns is still feeling the effects of Jacob Fatu’s assault from weeks ago, on his left elbow, and it hasn’t healed yet. Hence, there was no physicality between him and Fatu on the final RAW before the PLE in Turin.

It will be interesting to see if Jacob Fatu takes advantage of a possibly unfit Roman Reigns and becomes the new World Heavyweight Champion in WWE.
